A last minute referral
At around 10:30 pm which is the closing time of my clinic, a patient came with the complaints of giddiness & dizziness, palpitations and numbness if his left upper limb since 5 pm afternoon.
On examinations all of his vitals (Blood pressure, pulse, blood sugar etc) were normal. But seeing that it seemed that he was not OK at all. So I did an ECG and found some features consistent with IHD. Also there was long QTc interval which is a warning sign of life - threatening ventricular arrhythmia. My clinic didn't have facilities to manage the case so that I had to escort the patient to the government hospital.
