FOR THIS WEEK, HERE ARE THE CASES FOR YOU TO ATTEMPT.

Case 1

>A 49-year-old married woman who had a regular menses suddenly stopped 8 weeks ago, She initially thought she had attained menopause and so she decided to remove her contraceptive implants thinking she would not conceive afterward.

>Yesterday, she experienced a severe abdominal pain and also noticed a sharp pain at the tip of her shoulder, her body temperature was high and she vomited about 3 times.

>Early this morning, she was rushed to the hospital by her husband as an emergency-unconscious; her husband said she suddenly fainted while making breakfast 30minutes ago.

  • What do you think is this woman's medical problem (diagnosis)?
  • If you were a doctor, how would you help her recover?
  • What lessons have you learned from her story?

Case 2

>A 50-year-old chain smoker, who has been drinking for the past 2 decades suddenly started experiencing a slight increase in body temperature. This was the fourth time in 2 months he noticed this and had treated himself for malaria.

>He had noticed a change in his eye color earlier which he waved off as a symptom of malaria, but he couldn’t just fathom the sudden pain on the right side of his upper abdomen, he also noticed a fluid collection in his abdomen which increased as the day went by.

>Initially he thought it was an evidence of good living,( abdominal swelling), but when he noticed a similar swelling on both legs, he decides its time to finally consult a doctor.

Tell us about yourself

> I'm Bejide, Motunrayo Oluwabukunmi, from Southwest Nigeria, a microbiologist by training. I go by the username @bookoons on steemit.


Why do you love @air-clinic?

> Air-clinic has provided an avenue for medics, paramedics and even none medics to mingle and share knowledge, it is a home for all.


High and low point on steemit

> Low point were my early days, no matter how much I put effort into it my posts, I'd never get upvotes so I quit for about 2 months. High point was when I started my salon Beauty Parlour with @bookoons, I began to feel I was doing something.


How do you balance your steemit life and real life

>The challenges medical microbiologists have is the unacceptance in hospital labs by Medical Laboratory scientists, I've never had to experience that though because I've always been more interested in the research lab and there's always a space for a microbiologist with biotechnological background there.


Motivation to study your career.

>I've always loved genetics, molecular biology and anything that's related to genes and genomes. I needed no further motivation.


Challenges/survival story in your specialty

> I'm a social introvert, so even before steemit, my devices (phone or PC when I had one) were my best friends. I don't go out much and even when I do, I visit the same set of people. Steemit is an extension of my online self.


Final words

> I'm not sure I have a new thing to say to medics all over, you are in a noble profession, one that's so important though a lot of people don't appreciate it but keep doing all you can to make the world a healthy place.

For case 1,
my diagnosis is ruptured ectopic pregnancy.
Investigation urgent PCV, RVS, PT, GXM,Abdomino pelvic USS,...
Treatment is resuscitation (IVF, transfusion), then surgery.... Exploratory laporotomy.

For case 2

My diagnosis is chronic liver disease ( probably alcohol liver disease)
Investigations - LFT, SEUCR, Abdominal USS, Clotting profile, CBC

Treatment. Stop alcohol and cigarettes intake.
Treatment is symptomomatic and palliative( IVF, Antibiotics, Diuretics, laxatives...)

A liver cirrhosis/failure for the second case.
It's just fun to guess these symptoms , I am not a doctor anyway.
