Though our era is too much modern but here, patients are facing some critical problems in medical institute. Some times patients become helpless for irresponsibility of medical institute and workers of medicals. Today I am discussing about a decentralized application which come to us with a great solution and that is now available in blockchain system. ** Maximum time we become tensed about our health and diseases, so we are running from one Dr. to another. And maximum time when turn to another Dr then we will fail to provide our previous medical records.

** When we fail to provide our valuable medical records, as a result again those expensive tests are tested by Dr.

** If a patient become well and discharge from a medical institute, some time they loss their medical records so when these are needed, they fall a critical condition.

** Besides maximum medical institutes have no moder info recording system so when a patient back to them they can't provide their valuable medical records. Then a complex condition is created.

** Although some medical institutes keep their patient records but they haven't a system of exchanging these records. So when a patient go another institute they become too much helpless.

** In some critical diseases, so many tests are done. So sometimes patients can't keep their records instantly. So then they loss it.

** Sometimes the lacking of medical records, the Dr. can't give proper treatment to patients.

** Even some time if a patient loss his medical records, then Dr. give him some first step test but if the disease goes to final stage, it become too much difficult to give the patient any service.

** Some medical institute keep the patient's records on paper. So when it is needed then some time they can't find it.

** Besides some times for proper medical records, patient's life fall on a life risk.
