Morale Measurement in the Workplace

"MORALE: the mental and emotional condition (as of enthusiasm, confidence, or loyalty) of an individual or group with regard to the function or tasks at hand, a sense of common purpose with respect to a group, esprit de corps."
I spent some time developing this for my workplace which has struggled to implement a successful tool so far!
Each shift/department is scored in each area from: 0 - 1 - 2 - 3. The lower the score, the better overall Morale of the Workplace.
Workplaces with a high Morale reportedly have less mental health issues such as Depression.
Acronyms used:
SL = Sick Leave
FAI = First Aid Injury
LTI = Lost Time Injury
CDR = Customer Dissatisfaction Report
5S = Sort, Set In order, Shine, Standardize and Sustain

Morale is pretty important!
That's pretty neat! We used to have 5S as well when I used to be an engineer. Interesting to see it is used worldwide!
