A new concept
A few years ago, a new term was formally introduced to the world in medicine, it is the word "carpalgia" and its meaning is very simple: "pain in the wrist" (I am talking about the region before the hand, do not confuse it, etc...). Writes no more and no less, the creator of that term, Dr. Leopoldo Maizo, me! :D. And you may ask: how did the process go or how did I come up with it?
In one of my many doctor's guards during the postgraduate course in orthopaedics and traumatology, one of those typical in my country where you evaluate more than twenty patients in three hours, I observe several people with different ailments, I remember that the first of them is an elderly woman with pain in the lower back, so when I recorded her in history (a digitized system we have in the consultation) I put the appropriate medical term on her: "low back pain", another patient arrived with shoulder pain "omalgia", a child after a fall had pain in both knees: "gonalgia", a gentleman had a blow when falling from his bed in the coccyx: "coxalgia", a lady's foot bothered him a lot near the fingers: "metatarsalgia" and finally an elderly lady came to me who broke her wrist presenting incredible pain, but she did not know what she ended up using in her story, in fact it had not been apparently described... - "Strange..." (I thought). The doubt intrigued me, so I made the decision to look in the books and none of them helped me, since nothing was described... After that I decided to ask my professors and hand surgeons, taking by surprise the fact that they didn't know a term that referred to it either.

Un nuevo concepto
Hace pocos años se dio a conocer formalmente al mundo un nuevo término en la medicina, se trata de la palabra “carpalgia” y su significado es muy simple: “Dolor en la muñeca” (hablo de la región antes de la mano, no se confundan eh...). Ni más ni menos les escribe el creador de ese término: mi persona, Dr. Leopoldo Maizo. Y tal vez se pregunten: ¿cómo fue el proceso o cómo se me ocurrió?
En una de mis tantas guardias de médico durante el postgrado de ortopedia y traumatología, de esas típicas en mi país donde evalúas a más de veinte pacientes en tres horas, observo varias personas con diferentes dolencias, recuerdo que la primera de ellas una señora mayor con dolor en la región baja de la espalda, así que cuando la registre en la historia (un sistema digitalizado que tenemos en la cosulta) le coloqué el término médico apropiado: “lumbalgia”, otro paciente llego con dolor en el hombro “omalgia”, un niño luego de una caída tuvo dolor en ambas rodillas: “gonalgia”, un señor tuvo un golpe al caerse de su cama en el coccis: “coxalgia”, el pie de una señorita le molestaba mucho la zona cercana a los dedos: “metatarsalgia” y por último me llegó una señora de tercera edad que se fracturó la muñeca presentando un dolor increíble, pero no sabía que termino usar en su historia, de hecho no había sido aparentemente descrito… -“Que extraño” (pensé yo). La duda me intrigó, así que tome la decisión de buscar en los libros y ninguno me ayudó, ya que no aparecía nada descrito… Luego de eso decidí preguntarles a mis profesores y cirujanos de mano tomándome por sorpresa el hecho de que ellos tampoco conocían un término que le hiciera referencia.

And here in this part you will ask yourselves: And what does this matter? In the medical language it has a significant value for this specialty, not only the technicality of using it but also the way of generating simplified searches based on this term, that is to say, it is usually easier to search for pathologies related to "causes of low back pain" or probable manifestations and surgeries, than to search for texts that refer for example: "causes of back pain in the lower zone". It sounds really different and loses that focus that the doctor needs for communication between colleagues (the same thing happens in all professions).
So before I decided to propose any word at random myself, I decided to look for how those words that already exist are formed; for example "omalgia" (shoulder pain), I discovered that like all the others, they derive from a Latin composition of "omos=shoulder" and "algia=pain" and therefore "omalgia=shoulder pain". Understanding this and if doll in Latin is "carpal" and pain is "algia", the compound word would be "carpalgia" (ta-taaaaaaaaan... First step completed!).
¿Y de qué sirve?
Y aquí en esta parte ustedes se preguntarán: ¿Y qué importancia tiene esto? Pues en el lenguaje médico tiene un valor significativo para esta especialidad, no solo el tecnicismo de usarlo sino la manera de generar búsquedas simplificadas en base a ese término, es decir, suele ser más fácil realizar una búsqueda de patologías relacionadas a “causas de lumbalgia” o probables manifestaciones y cirugías, que buscar textos que refieran por ejemplo: “causas del dolor de espalda en la zona baja”. Suena realmente diferente y pierde ese enfoque que el médico necesita para su comunicación entre colegas (pasa lo mismo en todas las profesiones).
Así que antes de decidir yo mismo proponer cualquier palabra al azar decidí buscar cómo se forman aquellas palabras que ya existen; por ejemplo “omalgia” (dolor de hombro), descubrí que al igual que todas las otras, derivan de una composición del latín “omos=hombro” y “algia=dolor” por lo tanto “omalgia=dolor de hombro”. Entendiendo esto y si muñeca en latín es “carpo” y dolor “algia”, la palabra compuesta seria “carpalgia” (¡taa taaaaaan… Primer paso completado!).

Approval
The fact of proposing a new term for its acceptance is not as easy as it sounds and my first option in my mind was to send an email to the Dictionary of the Royal Spanish Academy (since at the beginning I only wanted to introduce the term in Spanish) where I explained the above to them, after a few months I had an answer that told me that neither they, nor anyone else could introduce a new word arbitrarily in the dictionary (it sounds obvious after all), to incorporate it you need at least 6 years of disclosure in recognized texts and by people of great prestige in the branch or specialty of that term.
But my desire didn't die and then I got in touch with the best surgeons in the Spanish-speaking world (Dr. Diego Fernández, Dr. Alejandro Badia, Dr. Marc García Elías, Dr. Nelson Socorro... among others), great personalities in different countries who provide their services, I proposed my idea to them and they were so enthusiastic that they continued to motivate me.
In June 2016, after sending a document to the Spanish Society of Orthopaedic Surgery and Traumatology (SECOT) in February of the same year, I was honored with the second prize for the best research work on pain, and with a monetary prize! The excitement was so great that I just couldn't believe it.
In September 2016, after a trip of more than 12 hours (by plane and then by bus from home) I presented my research work to orthopaedic surgeons from all over the world in "La Coruña" (Spain) before a high category Congress where I received my recognition.
In January 2017 the Spanish Journal of Orthopaedic Surgery and Traumatology published my work and since then thousands of people around the world have had access to it. It was widely accepted that today and with medical formality the term "carpalgia" was made and patented here, in my homeland, in Venezuela.
Sometimes those ideas that we think are too silly if we give them the courage they deserve will propel us beyond what we think they are worth. And you know... If your wrist hurts, go tell your doctor you have "Carpalgia". Hehehe.

La Aprobación
El hecho de proponer un nuevo termino para su aceptación no es tan fácil como suena y mi primera opción en la mente fue enviarle un correo al Diccionario de la Real Academia Española (ya que al principio sólo quería introducir el término en español) donde les expliqué lo anterior, luego de algunos meses tuve una respuesta que me decía que ni ellos, ni cualquier persona podían introducir una nueva palabra de manera arbitraria en el diccionario (suena obvio después de todo), para incorporarla se necesita de por lo menos 6 años de divulgación en textos reconocidos y por personas de gran prestigio en la rama o especialidad de ese término.
Pero mis ganas no murieron y luego ponerme en contacto electrónico con los mejores cirujanos del mundo de habla hispana (Dr. Diego Fernández, Dr. Alejandro Badia, Dr. Marc García Elías, Dr. Nelson Socorro… entre otros) grandes personalidades en diferentes países que prestan sus servicios, les propuse a ellos mi idea y se entusiasmaron tanto que me siguieron motivando a mí.
Ya en junio de 2016 luego de enviar un documento en febrero de ese mismo año a la Sociedad Española de Cirugía Ortopédica y Traumatología (SECOT), fui homenajeado con el segundo premio al mejor trabajo de investigación en dolor ¡y con premio monetario!!! La emoción fue tan grande que simplemente no lo podía creer.
En septiembre del 2016, luego de un viaje de más de 12 horas (en avión y luego autobús desde mi casa) presenté mi trabajo de investigación frente a cirujanos ortopedistas de todo el mundo en “La Coruña” (España) ante un Congreso de alta categoría y donde recibí mi reconocimiento.
En enero de este año 2017 la Revista Española de Cirugía Ortopédica y Traumatología publicó mi trabajo y desde entonces miles de personas en el mundo han tenido acceso a ella. Se difundió con gran aceptación que hoy en día y con formalidad médica el término “carpalgia” fue hecho y patentado aquí, en mi tierra natal, en Venezuela.
No tengamos miedo en superarnos nunca, a veces esas ideas que pensamos que son muy tontas si les damos el valor que merecen nos propulsaran más allá de lo que pensamos que valía. Y ya sabes... Si te duele la muñeca ve y dile a tu doctor que tienes "Carpalgia". Jejeje

No @mlaura, the carpal tunnel is a specific pathology, but the pain can give "carpalgia". In fact, carpalgia is applied to all kinds of pain in the wrist, including the carpal tunnel. Thanks for your interest and your words.
