Mistrust at McDonald's

So I bought my son a happy meal and upon glancing at the recipt I noticed a odd way the McDonald's entices it's customers to give a "Highly Satisfied" rating.

Is it me or does this seem wrong? Every other survive I have seen like this only ask for a rating and will usually offer something regardless of the rating.

I wonder how many unsatisfied "Highly Satisfied" customers have claimed their buy one get one offer ...
