Part of what makes New Jerusalem a paradise is the fact that it has been sterilized, so without harmful bacteria or insects, nothing degrades or deteriorates.

Matthew's statement was meant to suggest that those who showed themselves worthy, stored up figurative "meritorious capital" with The Lord, and part or their reward will be a very posh life in New Jerusalem.

New Jerusalem is the capital city of the entire New Kingdom, but just like Washington DC is the capital of the United States, the other states are separate but united governments under the federal government, so in the same way, New Jerusalem is the seat of authority over the entire New Kingdom, and the smaller kingdoms are subordinate to the overarching kingdom.

As for who will live in New Jerusalem, it will be a very select group, but of course, all of the clergy will have residences in the capital city, but they'll probably only be using them when they're visiting, since most likely they will be staying in their sub-kingdoms to oversee them.

The "Outer Darkness" is the vast wasteland area in between the kingdoms.
