It should be noted that this hallucination reaches such extreme and dangerous levels that make people who suffer from this type of mental disorder do not trust at all what their senses tell them, and base all their "rational" logic on the result that they are being attacked by an endless number of parasites and insects, to the point that they are capable of feeling the incessant need to scratch themselves to such an extent that they do so until they bleed.

So how do we differentiate whether this is a form of delusion or hallucination? Because the words "not trust at all" seems suggestive of saying that the patient is delusional rather than having a form of hallucination. Or is the hallucination experienced by people with Ekbom syndrome were so severe that they can't really differentiate between what is real and what's not?

Indeed, the syndrome of ekbom is a delusional disorder whose main and the most dangerous symptom is the hallucinations that it generates and that make the patients who suffer it tend not to be in control of their actions since they are taken to commit all kinds of irrationalities by the severe and invading hallucinations.

Or is the hallucination experienced by people with Ekbom syndrome were so severe that they can't really differentiate between what is real and what's not?

Those who suffer from this disease, cannot differentiate between what is real and what is not, hence the fact that by more tests and exams that patients receive these will continue to firmly believe that they have insects under their skin.
